Habilitation à exercer l'activité professionnelle:

— declaration on honour from applicant certifying that they have no conviction during the past five years entered in the “bulletin 2” of the national criminal record for offences referred to in articles L 8221-1, L 8231-1, L 8241-1 and L 8251-1 in the labour code;

— a copy of any court judgements if the candidate is in receivership;

— a declaration on honour that the candidate has not been banned from tendering according to the criteria defined by article 43 of public contract code;

— a declaration on honour that the candidate is in good standing in the year preceding the year in which occurs the launch of the consultation, under articles L. 5212-1, L. 5212-2, L. 5212 5 and L. 5212-9 of Labour code regarding the employment of disabled workers.

— if the candidate is established in France, a declaration on honour that the work is performed by employees legally employed under Articles L. 1221-1210, L. 3243-2 and R. 3243-1 of the Labour Code (in case the candidate has employees, in accordance with Article D. 8222-5-3 of the Labour Code);

— if the candidate is established or resident abroad, a declaration on honour certifying the issue of the pay slips to employees containing the information required under article R. 3243-1 of the Labour code or equivalent documents;

Situation économique et financière:

— a statement on the overall turnover and the turnover relating to the goods, services or public works involved in this contract, realized by the undertaking over the past 3 financial years(or form Dc2);

—list of the principal deliveries or services provided over the past 3 years, their value, their dates and public or private recipients. Evidence of deliveries and services provided shall be given , by the purchaser's certification or, failing this, by a declaration by the economic operator (or form Dc2);

— a declaration of provision of social declarations (ursaff, rsi…) issued by the social protection organization responsible for collecting social security contributions payable by the co-contractor and dated less than 6 months previously (article D8222-5-1°) and Liasse 3666 at December 31st of the previous year;

— or form NOTI 2 (fiscal and social situation at December 31st of the previous year) and social document dated less than 6 months previously.

Capacité technique et professionnelle:

— a statement of the average annual manpower of the service provider or contractor and the number of managerial staff for the last 3 years (or form Dc2).

Exécution du marché
Principales conditions de financement et modalités de paiement et/ou référence aux dispositions pertinentes qui les régissent:

Departemental budget. Payment by administrative order. Payment terms : 30 days (article 98 of the public contract code). Fixed price.
